Founded by Doctors of Physical Therapy, Evergreen combines a passion for senior wellness with a commitment to helping residents maintain dignity, comfort, mobility, and quality of life.

Unlike larger facilities, Evergreen offers a small, close-knit setting where residents are known as individuals, families are kept connected through regular updates and photos, and care is tailored to each person’s needs and preferences. Residents enjoy home-cooked meals, meaningful daily interactions, personalized mobility and exercise support, and coordination with home health and hospice services when needed.

At Evergreen, we believe senior living should feel like home—not an institution. Our mission is to create a safe, nurturing environment where residents are treated like family and families can feel confident their loved ones are receiving attentive, compassionate care.
