Brightmoor Hospice: Adding Life to Days, When Days Can No Longer Be Added to Life
Talking about the prospect of hospice is never a happy time for family members and friends – but when it’s time to talk end-of-life or life-limiting physical conditions, you want the best there is.
At Brightmoor Hospice, our medical team helps patients cherish every moment, with quality care from compassionate staff members – whether in-home, at an assisted living, a nursing home or in our state-of-the-art inpatient facility.
The kind of life-limiting illnesses that Brightmoor treats is far reaching. These include Alzheimer’s, cancer, cardiac and respiratory diseases, as well as liver or kidney disease, ALS and other degenerative or neurological diseases. To receive hospice, patients need to have a six month or less prognosis and must no longer be seeking aggressive or curative care for their illness.
Brightmoor has the ability to partner with skilled nursing facilities and assisted living communities to formulate individual care plans to meet each of our patients’ specific needs. Hospice services include medical equipment, supplemental medical supplies and medicines related to their diagnosis. Hospice is covered by Medicare, Medicaid and most private insurances.
In addition, Brightmoor Hospice offers a 12-bed, state-of-the-art inpatient facility for those who meet the requirements for general inpatient levels of care. This type of care is for patients who need symptom management that cannot be controlled in their current environment, such as uncontrolled pain; intractable nausea and vomiting; uncontrolled respiratory distress; severe bouts of agitation, anxiety or delirium; as well as advanced open wounds that require more than one person to complete complex dressing changes.
No matter what tailored arrangements patients and their families choose, the Brightmoor Hospice care team includes a full range of medical professionals: a registered nurse, certified nursing aide, a social worker, chaplain and trained volunteers to get patients and their families the care they need.
Brightmoor Hospice also offers monthly in-home palliative care to support treatment for chronic illness – and it’s also available in our 29-county service area. Palliative care focuses on treating distressing symptoms to increase comfort and quality of life. Patients with chronic illnesses who have a prognosis of two years or less can now receive palliative care at home while still receiving aggressive treatments for their conditions.
The palliative care program provides nursing visits and case management. Social work and chaplain services are also available. The palliative care team helps to coordinate care with the patient’s PCP and specialists.
